This is a very close replica of the first known card based on the original 3Dfx Voodoo chipset (later revision of this card). Equipped with 12 MB of EDO memory which is distributed in equal parts between FBI (500-0003-03) and two TMU (500-0004-02) chips. 2nd TMU represents a 2nd deck attachable card with its own memory.
This seems to be one of few configs Q3D drivers recognize as a native and to which Quantum3D company provided a retail pack.
Partial Retail pack provided as: the carrier card, 2nd TMU card, box, manual, driver on a 1.44" floppy.
